How to schedule an appointment:

Press 1 for the “scheduling” option
… If we are busy with other clients and unable to answer your call directly please leave a voicemail with the following information:
1) Full Name
2) Insurance Plan (Aetna, UMR, cash, etc.)
3) Phone Number (for call-back)
4) Email Address (to send intake paperwork)
… we will then contact you via phone call within 24 business hours (at the very latest) to confirm an appointment date/time that works best for you.
After your appointment is scheduled:
- We will send your intake documentation by email.
- We kindly ask that you complete this at least 24 hours prior to your appointment. This will include:
- Intake packet
- will need a copy or photo or your ID and the FRONT and BACK of your insurance card)
- Consent for treatment
- HIPAA/Privacy notice
- Payment/Cancellation policies
- Intake packet
- These forms can be quickly and easily filled out online including via mobile devices
- If any clarification or further discussion is needed, we will reach out to you directly within 24 business hours
